This resource includes visuals to help students make individualized choices, mark interests, and share their wants and needs following graduation from special education programs.
If you are a special education teacher at the high school level this is a mandatory document to include in IEPs from age 14 and up. Students with disabilities must begin to be introduced to transition planning and job exploration at this age.
Students can complete this assessment at the beginning of high school, and update at each IEP meetings, or this can be used just for students in the transition period between high school and graduation (commonly ages 18-22).
What is an ITP?
An ITP is an Individual Transition Plan for Adults in Special Education who are soon aging out and making plans for their future based on their own individual interests, wants, needs, and strengths. This prepares students for post-high school and gives them a voice in their future life.
